The Return of the King, disc 1, track 13:

The Eyes of the White Tower

[00:16-00:27] All Shall Come to Darkness: A Noble End

[02:42-03:23] Minas Tirith (Silver Trumpets)

[03:23-03:40] The Realm of Gondor (in Decline)
This theme is interrupted by the hybrid Threat of Mordor/Mordor Skip Beat before the theme develops enough to determine if it is Gondor's Ascension or Decline. Given the state of things in Gondor, the somber quality of the music, and the fact that it follows Gandalf's explanation of the decrepit White Tree, I would say it is The Realm of Gondor (In Decline).
[03:40-04:32] The Mordor Skip Beat accompaniment

[03:40-04:32] The Threat of Mordor accompaniment
The Annotated Score refers to this point in the score as "a martial line somewhere between an inverted Threat of Mordor and the Mordor Skip Beat."
[03:51-04:01] The Evil of the Ring (Sauron/Mordor)
From the Annotated Score: "Shore reaches over the new Threat theme and deposits his inky History/Evil hybrid, this time emphasizing the Evil.
[03:51-04:01] The History of the Ring
